I think your suspicions are correct.
We had our 1501A unit replaced twice last year on our 922LX running MPE/iX
4.0 due to this same annoying problem, (and it stared creeping up on us
again for the third time). At first I suspected power failures as well, but
it did not make sense to me. Never got satisfactory explanation from RC on
this. Luckily, it never happend on our second machine - 937 with HP1503B unit.
 
Have you checked Predictive? When this irregular problem occurs, its' PSLIST
produces something like this:
 
INFORMATIONAL(RC) - 11:01 PM, 02/09/96 - Pdev 4.5.3:HPC1501-HPC1511(05)
      Device Reject. S0=%5413:S1=%100100:LOG0845:X000.
 
Do you get similar message? Anyone with suggestions?
