I believe that I have seen this once when I had a duplicate SCSI address.
I don't think you can't even get to ODE to run mapper.
I was trying to install a 2nd disk (or was it a dds-3 (?)).
So I made it address 9 or 3 or something I felt sure wasn't used,
then I could boot to ODE, run mapper and get current SCSI addresses
for hardware attached. THen I could set its address to what it needed.
