Great survey!  I had received a paper copy but it got buried in the piles
of pending stuff :-(

The online alternative had a better chance of being pursued and, in fact, I
just filled it out and added this comment to the end:

Congratulations on having used a historical criterion for placing the
various items according to seniority (MPE first, followed by HP-UX, and
then NetServer).  This subtle courtesy is appreciated.  Good job!
