For me, I see those ENTRY_INIT,TEST  errors when it can't find a proper scsi 
disk to boot with.
So either paths not right, terminator not good, reset cables.....
(not likely a clear disk?)
Noting - I'm a programmer, not hardware, unless its my own.

ISL/ODE/MAPPER from a tape (or CD) would be good here.
That might(should) be on your SLT tape - so you could get ODE/MAPPER
(boot from ALT; goto ISL prompt; run ode; run mapper2)

You can download the HP9000 Offline Diagnostics Environment PA0409  CD
and burn your own - if you had a CD in your hp3k. (it has ODE)

hmm, I wonder if I can make a tape image of the same(?).
