I don't know of a "textbook" for MPE, but there have been a number of
books -- collections of papers realy - that might do.
 
1) Thoughts and Discourses on MPE Software, VESoft:  A collection of
   papers by Eugene Volokh and, in some editions, Robert Green.  The
   papers cover a number of aspects of MPE and software development and
   is recommended reading.  VESoft often gives away copies, and it you
   are a customer, I suspect they would send you one.
 
2) The TurboImage Handbook, Wordware:  Again a collection of papers, but
   devoted to the Image database system.  There are a number of this to
   learn about MPE in these papers.
 
3) HP3000 Bible, PCI:  Out-of-print, and outdated none the less.  Some
   of the papers are still relevant.
