Well, when IMAGE does a "Now you see it, now you don't", often the cause is
ITEM or SET level security.  Depending upon which password is used to access
the database items and sets seem to appear and disappear.  Only when logged on
as created and supplying ';' password can you be sure to 'see all'.

When you say 'DBG said they were already there...', I assume we are talking
about QUERY opening the base with a password which does not see the items and
DBG opening with a password that does, possibly the 'see all' situation.

The issue might be when the items were added a WRITE/READ access class was not
applied to the items which should have been?  The fix might be to update the
item security to match other items. 

The test would be to access the base as creator and use the ';' password.  If
the items are then seen, we assume item level security is the issue.

Removing the entries and then adding with accessible security would correct
the
problem.  Easier might be to change the security of the items.
