Yes, and it seems the file IDP.CONFIG.SYS is inaccessible or gone.
This results in the A.00.00 display (instead of RELEASE: B.40.00
and whatever USER VERSION was entered in SYSGEN MISC)...
Oh, by the way, RELEASE B.40.00 was already one of those releases
where the 40 in B.40.00 was chosen to "remind" of the 4.0 "name",
similar to C.45.00 (4.5) or C.50.00 (5.0) or C.55.00 (5.5) etc.
