According to the Quiz 5.09 Manual dated 12/88, the filecodes are:

        640     Compiled QDD data dictionary
        641     Compiled Quick screen
        642     Compiled Quiz report
        643     Compiled QTP runfile
        644     Nonportable subfile with minidictionary
        645     Quick 3.02 or 4.00 QKGO file
        646     Compiled GRAPHICS report
        647     Portable subfile with minidictionary
        648     Portable subfile data file
        649     Interim minidictionary file
        650     5.06 QKGO file
        651     Temp sort file for Quiz/QTP
        652     Compiled portable minidictionary file

That's all the manual has.
