We had a similar problem with Telnet some time ago.  Turns out that we had
hit the maximum limit for Telnet sessions.  We increased that number to a
very large number that we felt we would never reach and have had no
problems since.  We were on 6.5 pp2 at the time.  It might be worth
checking to see if you are at the limit.
