There are two main possibilities:

   1. the disk was never setup as volume 2 of $sys after the most recent reload/install,
      but had been part of $sys in a prior life (which is what several people said)

   2. the disk was part of the current $sys, but something happened, and it now
      isn't mountable ... in which case you should have seen a message during bootup about it.
      
Although it's possible to determine which case is likely, it involves checking the UFIDs on
both LDEV 1 and 2 (to see if their timestamps are the same).  If you're curious, post the result
of:

   (logon as SM user)

   :debug

   dsec 1.0, $80, b

   dsec 2.0, $80, b

and I'll try to look at them and comment.

In either case, you're likely to want to SCRATCHVOL and NEWVOL LDEV 2, as suggested.

BTW, it is possible to get the SYSGEN tape to have the fact you want LDEV 2 as part of $sys,
but that's too long to write in the margins of this email.
