There's nothing in what you posted about the text description of the patch
to imply that it would have any relation to your program abort.

I wonder who generated the "[VSM]..." message! However, the instruction at
address: tl_open_mpe_ksam_file+$138 on our 6.0 system is not capable
of generating a trap. This makes me wonder:
What release of MPE/iX are you on?
And, is your program using Transact? (the language)
Also, has it worked before? What changed?
I'd investigate: are you trying to open an existing file when the
trap occurs, or a new file?
