Networked 2564 problem

I am trying to set up a 2564C with a parallel card on a Jet Direct box.  I can't remember the configuration settings for the
2564, and am 7000 miles (and 12 time zones) from my other (working) printers.  Can someone please tell me the configuration
settings for the printer, and the Jet Direct box?

I have placed a call to the HPRC, but so far no call back.  We want to get this working and go enjoy the week-end in China.
