I'll second the recommendation of OCS's "Librarian"
product.  It runs on both MPE/iX and HP-UX but I'm
pretty sure it doesn't handle the AS/400.  I've used the
product for over 7 years, and it's very flexible (I'm
actually updating one of my clients systems right now
that has been going strong for about six years now
with very few modifications outside of S/W updates
from OCS).  OCS technical support has also been
very good over the years when I've needed them.
They have been very responsive about product
enhancements or recommendations on features
I didn't think of using to get around a problem area.

Now if they would only port it to Windows NT, I'd be
a very happy "Librarian" camper :)
