I saw it frequently when 5.0 was new, with $OLDPASS being slow to
appear at times.  (When you close $NEWPASS, it should be immediately
renamed $OLDPASS, so that the next process you run can open it as $OLDPASS.)
In some cases, it seemed to take several seconds.  This drastically affected
a "compile" utility I use, so I had to add code to do:
 
   1) is $OLDPASS there yet?
   2) if not, pause (1 second) and go back to (1) (up to 10 times)
 
I reported the problem informally to the lab, but can't remember if I ever
filed an SR (shame on me!).
