The punch holes you're referring to were used when a sales rep was
designing a form with the ruler.  The 3/16" holes were used to draw
pinfeed holes down the side of the forms (on computer forms that were
fed in to a line printer on a continuous form).  The other two sets of
punch holes were for file punch holes the customer wanted punched in
their forms allowing them to use binders with two hole posts.  Also in
many cases the larger holes were used to draw punch holes to be used
in register machines.
