I've tried filling out this form in both Netscape and MSIE,
and I get Internal Server Error 500, contact the webmaster.
(O.K. I left a voice mail.)
One thing in noticed is that you cannot select any of the
MPE tracks in Netscape (although it showed up after the
error when I hit the 'back' button.)
