The IA and BA for yourgroup do not come from the "ALTACCT" but are the
defaults for the group when the group is created.

The last command (4) is incorrect, and should have been

ALTGROUP whatevergroupyourprogisexecutingfrom;CAP=+PH

The "+", means add the capability to the existing ones, without it, all
others ("IA" and "BA" etc), are removed, and you are left with just PH.

As a reminder, groups are created in an account, not the other way around.

The default capabilities for new accounts are as follows:

AM, SF, ND, IA, BA

I notice that "AM" was missing from your altacct(2), this could cause a few
problems.
Probably better (or at least less typing) for your ALTACCT would have been:

ALTACCT accountname;CAP=+PH

So perhaps I get the IA and BA from the "ALTACCT" command?  But if
that was true, would other accounts in my group get IA and BA?
