I think Gary's right. According to the HPSWINFO file on my 6.5 PP3 systems,
it's MPELXJ2A:

> MPELXJ2A  MESA diagnostics (CSTM) can cause mirrored disks to disable

** NOTE: ITRC shows this patch has been superceded, latest patch is
MPEMX77A. Other MPE versions (6.0, 7.x) will have slightly different patch
ID's. **

The ITRC web site description for patch MPELXJ2A says:

> The Mesa diagnostics (CSTM) call io_do_scsi_idy to identify data
> from disks and tapes.  This request goes directly to the Device
> Adapter Manager (DAM) and bypasses the Device Manager (DM), causing

If I had to guess, I would say the same problem affected SYSINFO's IOMAP
routine. If so, it appears to have been fixed.
