We found the problem yesterday, on the A.01.04 version of DISCUTIL.
From the ISL> prompt, an "LS" command shows:

filename type start size created
=====================================================
ISL -12864 493664 306 00/09/29 15:01:08
START -12864 4080416 51000 00/09/29 15:01:35
DISCUTIL -12864 1453232 20568 00/09/29 15:01:45
...

I don't know if all copies of A.01.04 have the creation date of
00/09/29.

Here's what happens when you try to give that version of DISCUTIL
a 36 GB disk:

ISL> discutil
MPE/iX launch facility

Initialize_genesis - Version : <<870204.1552>>
TUE, JAN 16, 2001, 7:01:31 PM (y/n)?

[CDM]11 1 4 10
Initialize memory manager completed.
*NOTE: VOLUME MOUNTED ON LDEV 1
*NOTE: LDEV 31 CONFIGURED AS DISC (PATH: 52.1.0).
*NOTE: LDEV 32 CONFIGURED AS DISC (PATH: 52.2.0).
(47/128) Continue?
*NOTE: LDEV 7 CONFIGURED AS TAPE (PATH: 52.0.0.0.0.0.0).
*ERROR: UNABLE TO MAP DISC AT LDEV 31 INTO VIRTUAL OBJECT - VOLUME NOT MOUNTED
*NOTE: VOLUME MOUNTED ON LDEV 32

Note that it refused to "mount" ldev 31, our 36 GB disk. It did successfully
mount ldev 32, our 18 GB disk.

This has been reported to the HPRC, as Call ID 3100522409

BTW, I'm also submitting an enhancement request to improve the user
interface of DISCUTIL :)
