Have a look at http://www.ep.cs.nott.ac.uk/pdfcorner/ where Phil Smith, the
author, has a 'free' c program, including source, that will do this.
You might have to increase the size of the 'pageObs' and 'locations' arrays.
We found that they were a bit too small for the files we were converting,
pageObs was increased to 500 and locations to 1000. The gcc compiler from
GNU was used to re-compile the source.
