IIRC, MPE/iX (and MPE XL) has always been able to bootup even if missing
a disk (or more) of MPEXL_SYSTEM_VOLUME_SET (well, except for missing LDEV 1 :)

...probably indicates that one or more extents of the file were on the
drive that's dead.

How do you mean that?  That the *labels* weren't on the drive?  Or that
not a single extent was on the drive?

Offline:

   DISCUTIL (it has a "SAVE" command)

   Once saved, and the $SYS set is re-created, the files can be pulled
   from the tape via VOLUTIL's RESTORE command.

   STORE <just the files that have no extents, no label, on the dead drive>
