Have  you recently recompiled your form file?

There is a "terminal selection" menu in Formspec that, if specified
inappropriately, could negate the "security" enhancement type that you may
have associated with your form.

Make sure you x-out all possible HP terminal types to ensure that the
"security" enhancement feature IS enabled.

I'm assuming the issue is a formspec one and is associated with the use of
this security enhancement feature of HP terminals.

It could be unrelated but can't think of anything else that could be
involved here.
