Since this is essentially a "silent data corruption" bug (the
worst possible kind), it would be nice to know under exactly what
circumstances the bug can occur, and which operating system and/or
patch levels this problem affects.

I know customers who would change OS versions at once to avoid even a
slim chance of running into this kind of bug.
