Well, I know what's happening.  The 'HIDDEN' fields (I've since added
NOENTRY, to no avail) are appearing in the middle of the screen.  What do I
need to do to get a default value in those fields but not allow anything to
be entered?

The field FILE-NAME is overwriting the label "File Name".
