I recently experienced the same thing.  Unfortunately after finally
installing the subsystem, and still being unable to properly configure the
card,  I was informed that when buying hardware from an HP VAR it is
wise, if not necessary, to contact the HP Software response center and
inform them as to what you're installing.  They will more than likely cut
you a DAT tape with the necessary patches to complete your installation.

We are installing 100Base-T cards in a couple of HP3000s.
With the cards came a subsystem tape to be installed but no intstructions
on
how to install the subsystem.  Called the response center and they said
they
would "try" to find some instructions to fax.  So far I haven't received
anything.
