You need to use the escape sequence to set the vertical motion index
(VMI) directly. This is <esc>&l###C, where ### is the line spacing in
48ths of an inch. So to get 66 lines in 7.5 inches (with a .5 inch top
and bottom margin), use <esc>&l5.45C. (because (7.5 / 66) * 48 =
5.4545...).
