I've seen this sort of thing happen (with DDS drives) if there was any
activity taking place on the drive (such as a tape load) during a "start
norecovery".
Could that have been the case in your situation? If possibly so, a simple
reboot ensuring no concurrent tape drive activity could be what's required.
